The following forms are due by January 31, 2021

  • Club Public Relations Effort AA4H4

The following are due by October 1, 2021

  • Proof of acceptance/rejection of IRS 990-N e-postcard filing for July 1, 2020-June 30, 2021 reporting period-Tax year 2019 (forward acceptance to April via e-mail)  
  • Club Treasurers Book with a copy of your Club Bank Statements (July 1, 2020-June 30, 2021)
  • Annual Financial Summary (FY-2020 July 1, 2020-June 30, 2021) (Make sure ALL required signatures are completed)
  • Club Annual Property Inventory Form (FY-2021 July 1, 2020-June 30, 2021)

The following form is due by November 15, 2021 with each Benefit Auction Item Donated

  • 4-H Donation Form

The following form is due 30 days prior to planned fundraising event

  • Club Fundraiser Approval Request Form

The following forms should be used for club field trips and special activities.  (Printed copies can be requested from the extension office.)

  • Liability Release and Informed Consent Agreement - All ages
  • Permission to Travel To/From a 4-H Event
