Here is where you will find all of the up-to-date forms, including those to accompany entries into spring shows and summer fair events.

New 4-H member enrollment forms are to be completed by the enrolling member and then signed by a parent/guardian and the club leader, before they are sent to the Extension Office.   The Donor Form is used for donations to the annual Benefit Auction as well as for all donations made to the Cecil County 4-H Youth Development Program throughout the year.
