Wicomico County Record Books
Wicomico County has developed new Record Books for 4-Hers to use. We hope these new forms are more fun and appealing than the ones in the past.
These record books have been created so they may be filled in on the computer, saved (if you have adobe professional) and then printed. However, you always have the option to print them out and fill them in by hand.
These record books are fully fillable and are in PDF format. Adobe PDF's are read and filled using Adobe Reader and is available for download. Click the here to download. If you have questions about filling out the PDF's please let us know.
The record book can be downloaded as one full document or by individual pages. Just click on each link below and the file will open up in a new window.

Record Book Tips
- On many of the pages there are boxes, some are check boxes, so all you need to do is click in the box and a check mark will appear.
- You are not limited on the amount of text you would like to write. When typing the text areas will resize your text to fit it to the space.
