Record Book Forms & Guidelines
Check out the new Record Book requirements! The title page and MD 4-H Summary Record have been combined into one packet. You no longer need to include the yellow sheet as the first page. Remember to check with your club leader to see when they will be collecting your club's Record Books in the Fall.
