Allegany County Fair Forms:

2012 Fair Book Design Contest - Due March 1, 2012.

Indoor Entry Form (available 2012) - If you need more room, attach this Extra Entry Form to your entry form. Do not mail or bring these forms to the 4-H Office. They must be returned to the address on the form or dropped off at the fairground office.

Allegany County 4-H Scholarship Owl reading

Allegany County 4-H Scholarship forms are available at the Extension Office. The decision for granting of scholarships shall be the responsibility of the scholarship committee. This committee shall base decision on the following:

1.     Number of years as a member of Allegany County 4-H Program.

2.     Leadership abilities and resourcefulness.

3.     4-H background, service to others, and quality of 4-H activities.

4.     Anticipated college expenses.

5.     Recommendations from a leader and a teacher or a counselor.

6.     Recommendation of the Allegany County Extension Educator, 4-H.

7.     Scholastic record from high school and/or college records.

A complete first-year application shall include:

1.     A completed application form which includes a summary of 4-H activities and is verified by a parent, guardian, or a 4-H leader and the Allegany County Extension Educator, 4-H

2.     A recommendation by a 4-H leader.

3.     A recommendation from a teacher or a school counselor.

4.     The recommendation of the Allegany County Extension Educator (4-H), including verification of applicant's involvement in 4-H activities.

5.     Picture of self.

6.     A transcript of high school grades.

7.     Certification of semester hours enrolled for next semester (13 or more).

8.     Signature of parent or guardian will be required for applicants under the age of 21.

Allegany County New Scholarship Application

An application for renewal includes:

1.     A completed renewal application.

2.     The most current transcript or copy of college grade report.  If current grades have not been received at time of application, prior semester grades must be submitted.

3.     Certification of semester hours enrolled for next semester (13 or more).
